Contracted Insurance Companies
Below is a list of the insurance companies we are contracted with and will bill directly for you after each procedure. If your insurance company is not listed, we will still bill them for your exam just as we would for any of our contracted insurances.
Please note that just because we are contracted with your insurance company does not mean that your plan covers the procedure that your physician has ordered. Please call your insurance company to find out if your plan covers your procedure or if the procedure needs to have pre-authorization.

We gladly accept Checks, Money Orders, VISA, MasterCard, HSA in the mail. We can also take your credit/debit card payments over the phone by calling 907.452.4777
